(THE TOOL TO ASSESS FRAGILITY IN THE RENAL PATIENT).
The increase in life expectancy in recent decades, both in Spain and in
developed countries in general, is associated with the increase in the number of
patients with advanced chronic kidney disease (ACKD). Given this fact, it is
increasingly common for the nephrologist to need to assess whether or not this
type of patient is a candidate for renal replacement therapy (RRT). In this
context, this app was born, which develops the European Renal Best Practice
Group Clinical Practice Guidelines, with the aim of helping doctors define their
patient's situation in terms of survival, CKD progression, quality of life and
in making decisions about the behavior to follow.
METHODS: The assessment is made by calculating
the Kidney Failure Risk Equation, Bansal Score, and Clinical Frailty Scale, the
results of which are used in a final algorithm that guides the nephrologist in
decision-making. In addition, the app has numerous other additional tests that
help to personalize the assessment for each patient.
RESULTS: Mortality Risk Prediction and ESRD
Progression Risk Prediction are assessed for shared decision-making such as
nephroprotective measures, advanced care planning, current or future RRT need
assessment, assessment in this second case, if counseling is required prior to
dialysis (modality selection, including conservative measures) and finally, risk
prediction when considering whether or not to start RRT. about the topic; and
formulas used to calculate the results obtained with their references. In

CONCLUSIONS: In a growing context of significant aging of the population
with CKD, where the concept of frailty is increasingly present in our specialty,
this application was born as a new tool that helps the nephrologist to define
the situation of his patient in in terms of survival, quality of life and
decision-making about the conduct to follow.
This app is a tool that can never replace clinical judgment.
